R. Bruce Wallace, III, M.D., F.A.C.S. is founder and medical director of Wallace Eye Surgery in Alexandria, Louisiana. He is Clinical Professor of Ophthalmology at LSU Medical School and Assistant Clinical Professor of Ophthalmology at Tulane School of Medicine in New Orleans.

After completing ophthalmology training at Tulane School of Medicine, where he served as chief resident, Dr. Wallace was certified by the American Board of Ophthalmology and the American Board of Eye Surgery. A member of numerous professional organizations, Dr. Wallace is a past president for the Society for Excellence in Eyecare and the American College of Eye Surgeons and serves as an editorial board member for Ocular Surgery News, Ophthalmology Management, EyeWorld, Review of Ophthalmology, The Video Journal of Ophthalmology, Refractive EyeCare, Advanced Ocular Therapy and Cataract and Refractive Surgery Today. He has authored numerous articles for medical journals and book chapters and edited the textbook Refractive Cataract Surgery and Multifocal IOLs.

Dr. Wallace has served as a primary investigator and consultant for a number of investigational intraocular lenses, including six multifocal IOLs and one accommodating IOL. He has directed annual meetings of the American College of Eye Surgeons and has presented courses for the American Academy of Ophthalmology and the American Society of Cataract and Refractive Surgery. Dr. Wallace was recently named one of the 50 Most Influential Ophthalmologists by a leading publication, Cataract and Refractive Surgery Today. He is currently serving as program committee chairman for the cataract section for the annual meeting of the American Academy of Ophthalmology.

As an early proponent for outpatient ambulatory eye surgery, Dr. Wallace planned and opened his own ophthalmic surgery center over twenty years ago. He is cofounder of Laser and Surgery Centers of America, an ASC development and management company.

Dr. Weston, a board certified eye physician and surgeon, received his Doctorate of Medicine and eye surgery training at Hahnemann University in Philadelphia. He was named Associate Professor and Residency Training Coordinator there and for six years taught and researched in areas related to lens implants for cataract surgery and laser applications in eye care.

Dr. Weston founded Vision Surgery & Laser Center and the Oregon Laser Eye Center (OLEC). He introduced the now standard No Needle, Not Stitch Cataract procedure to Southern Oregon. At OLEC he preformed the first laser vision correction procedures and as Medical Director, taught this technique to many other surgeons in our region. He was the first in the Northwest to use endoscopic laser for glaucoma and multi-focal lens implants. He continues to offer the broadest array of options for vision correction. He is a Member of the American Society of Cataract and Refractive Surgeons, a Fellow of the American College of Surgeons and a Diplomat of the American Board of Ophthalmology and is one of the small number of ophthalmologists certified in Cataract Surgery by the American Board of Eye Surgeons. He is past President of the Douglas County Medical Society and has received the Helen Keller Benefactors award and Melvin Jones Fellowship as part of his efforts to restore vision through Lions Club International.

Pravin U. Dugel, M.D. graduated from UCLA School of Medicine, Los Angeles, California, in 1988. He was an intern at LAC/USC Medical Center and completed his residency at the Doheny Eye Institute, USC School of Medicine, Los Angeles, California, in 1992. Dr. Dugel completed Fellowships in Vitreo-retinal Surgery at the Doheny Eye Institute and in Vitreo-retinal Diseases at the Bascom Palmer Eye Institute, University of Miami, Florida, in 1993.

Dr. Dugel was the Chief Resident Supervisor at the Doheny Eye Institute until he joined Retinal Consultants of Az, Ltd. in July 1994. He has been co-investigator in National Eye Institute research studies and in several Multicenter Studies. Dr. Dugel has authored more than thirty papers and book chapters and is the first person to receive both the Heed Foundation Fellowship Award and the Ronald G. Michels Vitreo-Retinal Surgery Fellowhip Award.

William J. Fishkind, MD, FACS is a Clinical Professor at the University of Utah in Salt Lake City Utah, and the Director of the Fishkind and Bakewell Eye Care And Surgery Center in Tucson, Arizona. He is also a Clinical Instructor at the University of Arizona in Tucson. He attained his medical degree from Tufts University School of Medicine. He is board certified in both internal medicine and ophthalmology.

He completed an ophthalmology residency at a combined program of the USPHS Hospital and Louisiana State University in New Orleans Louisiana. Dr. Fishkind is a Fellow of the American Board of Ophthalmology and has been awarded an honor award. Additionally he is a Fellow of the American College of Surgeons. He is a member of the program committee and government relations committee of the American Society of Cataract and Refractive Surgery, and the cataract committee and skills transfer advisory committee of the AAO. He is immediate past president of the Outpatient Ophthalmology Surgery Society.

Presently, Dr. Fishkind travels extensively teaching both nationally and internationally. He has been a visiting professor at many universities, given many scientific presentations. He has lectured and served as an instructing guest surgeon throughout the United States, as well as in Ireland, Spain, Italy, Japan, Malaysia, Thailand, China, and Korea.

He has edited a textbook, Complications in Phacoemulsification; Avoidance, Recognition, and Management published by Thieme Publications in New York City. Additionally has authored many articles as well as book chapters on phacoemulsification and intra-ocular lens implantation.

He is honored to be one of Americas Top Physicians and Tucson’s Best 100 Doctors for the past 5 years. He is listed in Marquis Who’s Who in America.

Dr. Jerome Levy is a Magna Cum Laude graduate of Syracuse University, where he earned membership in the Phi Beta Kappa Honor Society. He received his medical degree from SUNY Downstate Medical Center in 1966. He completed an internship at SUNY Downstate Medical Center and Kings County Hospital in 1967. He served as Chief Resident in Ophthalmology at Manhattan Eye, Ear and Throat Hospital from 1968 to 1971, Coordinator of Phacoemulsification Training from 1972 to 1975 and Chief of the Laser Clinic from 1975 to 1990.

Dr. Levy has been an attending surgeon at Manhattan Eye, Ear and Throat Hospital (now Lenox HillHospital) and the Ambulatory Surgery Center of Greater New York for more than 30 years. He has been the author of numerous international articles and book chapters, given countless lectures, and was among the first physicians to introduce the Yag laser in the United States from Switzerland in 1982.

Dr. Levy’s specialized training includes phacoemulsification surgery with intraocular lens implantation, laser surgery, and treatment of glaucoma and diabetic retinopathy. He has performed thousands of refractive surgeries, including LASIK.

In 1987, Dr. Levy co-founded the Ambulatory Surgery Center of Greater New York. He serves there as President, Medical Director and attending surgeon. The Ambulatory Surgery Center is a Medicarecertified same day surgery center licensed by New York State. Since 1990, it has earned the accreditation of the prestigious Accreditation Association for Ambulatory Health Care.

Dr. Levy Co-founded New York Eye Surgery Center in 1987. He currently serves as President and Surgeon Director and he and his associate ophthalmologists provide high quality eyecare to more than 25,000 patients each year.

Stephen H. Johnson received his Doctor of Medicine degree from the University of Minnesota. He completed an internship in Internal Medicine and his residency in Ophthalmology at the Mayo Graduate School of Medicine, Mayo Clinic, Rochester, Minnesota.

Following residency, Dr. Johnson completed a fellowship in cataract and intraocular lens surgery. His continued interest in this area has resulted in the authorship of numerous articles in peer reviewed journals, chapters in multiple text books as well as intraocular lenses and instruments which bear his name. He has taught and demonstrated cataract surgery domestically from coast to coast and internationally spanning countries from Europe to China. He has also served as a medical missionary with the Lighthouse for Christ in Mombasa, Kenya, East Africa.

Dr. Johnson is board certified by the American Board of Ophthalmology. He is a fellow of the American Academy of Ophthalmology and is the recipient of their Honor Award given in recognition of his teaching his fellow ophthalmologists. He was also chosen by the AAO to be one of the authors of their current Maintenance of Certification materials from which the current board certification questions are drawn.

Considered a founding member of the American Society of Cataract and Refractive Surgery, Dr. Johnson has served on its Scientific Advisory Board as well as its Cataract Clinical Committee. He is honored to have been chosen annually by his peers to have been listed in “Best Doctors in America” every year since its inception in 1992.

Richard Hoffman, M.D. (2013)Richard Hoffman, M.D.
A native of south Florida, Dr. Hoffman completed his undergraduate degree in Molecular Biology at Haverford College in Pennsylvania in 1983. He received his medical degree at Tulane University School of Medicine in New Orleans, Louisiana, in 1987. He completed his internship at Presbyterian/St. Luke’s Medical Center in Denver, Colorado. After an additional year practicing medicine in Colorado, he returned to Tulane University for his residency training in ophthalmology. Following residency, he completed a year of highly specialized training in Refractive Surgery and Cornea and External Diseases in Louisville, Kentucky. He became certified by the American Board of Ophthalmology in 1994 and recertified in 2004. He has extensive training and experience in corneal transplantation and excimer laser refractive surgery.

Dr. Hoffman has over 300 publications in numerous ophthalmic journals and textbooks. He has performed live surgery internationally and has made hundreds of presentations at both international and national academic meetings and instructional courses. He has won numerous “Best of Session” awards for his presentations and “Best of Show” awards for his teaching video productions. In 2008, his video Scleral Fixation Without Conjunctival Dissection won First Place in the New Techniques category of the American Society of Cataract and Refractive Surgery Film Festival. He is a column editor for Eye World magazine and Ophthalmology Times and he is on the editorial board for the Video Journal of Ophthalmology. He is a member of the prestigious International Intraocular Implant Club. Dr. Hoffman has been a clinical investigator for multiple FDA clinical trials. He has designed new surgical instruments and techniques for anterior segment surgery and holds an academic appointment of Clinical Associate Professor of Ophthalmology at the Casey Eye Institute, Oregon Health & Science University.

Dr. Hoffman has a personal interest in computer-interfaced musical composition and recording in his home studio. He enjoys skiing, scuba diving, and spending time with his wife JoAnn, sons Noah, Zachary, and Jacob, and their daughter Macy. He has a portuguese water dog named Maluko.

I am the senior partner of a group of 15 anesthesiologists covering 4 ASCs in New York and New Jersey. We now do 28,000 cases per year and have been growing every year. I began 25 years ago in the first ASCs in New York State with Dick Mackool, Hal Schneider and Marvin Kochman when I joined the founding partner of my group Marty Livingston who had been at the NY Eye and Ear Infirmary for many years before being asked by Dick Mackool to help in his new surgery center.

Our group has real expertise in ophthalmic anesthesia for adults in the ASC setting. In addition we have been involved in the set up and development of ASCs as well as the ongoing operations. We are sometimes a very good neutral party when there are differences of opinion between major players.

In addition to my ophthalmic work I have been a cardiac anesthesiologist at NYU for the last 25 years one day per week where I do anesthesia for heart surgery and teach residents, fellows and medical students.

Robert Nelson, PA-C, is a board-certified Physician Assistant and the Executive Director of Island Eye Surgicenter on Long Island, New York. Island Eye Surgicenter is a three-operating room facility with 35 attending physicians, who perform nearly 10,000 procedures annually.

Prior to joining Island Eye, Mr. Nelson served in similar administrative roles at other single specialty and multi-specialty ASCs in the New York metropolitan region and in Connecticut. He has practiced as a surgical Physician Assistant for more than 32 years in areas such as general surgery, orthopedics, and vascular surgery. Mr. Nelson brought his extensive experience to the field of ophthalmology in 1993 and has never left.

In addition to his day-to-day management responsibilities running a busy ASC, Mr. Nelson still finds time to keep his PA skills up to date in the operating room, assisting on cornea transplantations, glaucoma cases, and oculoplastic and reconstructive surgical procedures every week. It is that combination of clinical experience and management expertise that affords Mr. Nelson the unique qualities needed to understand and run an efficient ASC. In addition to his responsibilities maintaining local, state, and federal statutory compliance, he coordinates numerous pharmaceutical and device research studies conducted by Island Eye surgeons. He has published many articles in industry magazines.

Mr. Nelson is a graduate of the Physician Assistant Program at the Harlem Hospital Center, which was affiliated with Columbia University College of Physicians and Surgeons. He holds a Bachelor of Science degree in the Medical Sciences from The New School in New York City

Kent Jackson, PhD., CPCU, owner of Jackson Research, Strategy, Solution, LLC, provides strategic planning, branding and member research and development support for OOSS. As an independent consultant he also serves a select range of long-term clients in the medical, insurance, education and real estate industries. In association with other qualified firms and individuals specializing in graphic and web design, copywriting, advertising, public relations and market research, Kent has developed refined methodologies and services enabling clients to strategically position and sell through their distinctive value advantage. For OOSS, Dr. Jackson leads the annual Benchmarking Initiative and industry-wide Ophthalmic ASC Strategic Leadership Survey.

Dr. Jackson’s 40+year career uniquely qualifies to address custom market and organizational research, business strategy and solution challenges. For the past 7 years he has worked closely with OOSS programs and initiatives, guiding the society’s branding and strategic planning process, member research, clinical and business benchmarking and membership development programs. Prior to starting his own firm, he served as principal and director of research for Lance Jackson & Associates (LJA), a graphic design and branding company. Prior to joining LJA he was employed by State Farm Insurance for ten years as a facility manager, regional planning director and internal marketing and sales consultant. He was first introduced to State Farm while managing a challenging project on their behalf and as co-owner of the Baker Regan Group, Inc., a full-service construction engineering and planning firm serving northern Colorado. For Baker Regan he served as marketing director and led client research, planning and project management for a host of building and development clients including private owners, corporations and institutions. He began his career as a university faculty member and administrator. He earned bachelor and master’s degrees from the University of Northern Colorado in 1967 and 1968 respectively and the PhD from Oregon State University in 1979. Hi academic and research focus has included the visual arts, industrial arts, psychology, human and organizational development and leadership. He currently serves on the graduate faculty of Regis University, is an instructor/facilitator for the CPCU Insurance Leadership Institute and provides services to a select client list of for-profit and nonprofit organizations in the medical, insurance, education and real estate industries.
